Dependent Verification Process Underway
The PAMCAH-UA Local 675 Health and Welfare Plan (“the Plan”) provides some of the most comprehensive benefits in the construction industry. To protect the integrity of the Plan and ensure that only eligible dependents are covered, a Dependent Eligibility Verification will take place over the next several weeks.
Part D Advisors (PDA) has been contracted to conduct this process. You will receive a verification form in the mail from the PDA Verification Team, along with instructions and a list of required legal documents.
We recommend that you begin assembling your documentation now to help ensure you meet the verification deadline. Examples of required documents may include:
- Marriage certificate or the first page of your most recent income tax return
- Birth certificates for your children
- Court documents for adopted children or children of divorce
- Medical proof of incapacitation for dependents covered due to total disability
Participation in this verification is mandatory. If the PDA Verification Team does not receive your completed form and supporting documents by the deadline listed on the form, coverage for your dependents will be terminated.
Verifying dependent eligibility helps maintain the strength of our Plan and manage healthcare costs for all members. Your cooperation is greatly appreciated.
